小児期発症慢性疾患患者の成人移行支援が推進される中、成人診療科での支援の実際は報告が少なく明らかではない。小児科から成人診療科へ切れ目ない支援を実施し診療中断や転科困難を減らし効果的な自立支援を進めるために、成人診療科での成人移行支援の現状を明らかにし、小児期発症慢性疾患患者の障害及び特徴に合った多職種による成人診療科での移行支援のガイドラインを作成する。
