Outline of Research at the Start

「地域包括ケアの世紀」では、高齢者が住み慣れた土地で生活できる視点がより重要視され、地域の「力」を活用し、住民参加型の介護予防をフォローアップできる体制を構築することが期待される。本研究では、介護予防の自主グループ参加者のニーズを明らかにすることにし、活動のプロセスにおけるニーズに応じて看護職がフォローアップするために必要な要素・内容を抽出し、フォローアップガイドを作成し、ガイドの内容の妥当性を検証する。
