Outline of Research at the Start

ポリファーマシーは、認知機能の低下、入院や死亡リスクの上昇等と関連が報告されており、減薬を目的とした服薬管理だけでなく、多疾患併存や老年症候群など加齢に起因する問題も含め総合的に対策を講じる必要がある。訪問看護師は、多疾患併存である地域高齢者の医療と介護の側面から在宅療養を支え、服薬管理を介した総合的な看護を提供している。本研究では、地域高齢者のポリファーマシーに対し医療介護レセプトデータを用いて、訪問看護サービスを中心とした種々のケアサービスによるアウトカム評価を行う。リアルワールドデータ解析により、地域高齢者のポリファーマシーに対し、効率的なケアサービスの提供体制の整備に寄与すると考える。
