Outline of Research at the Start

本研究の目的は、応急仮設住宅で暮らす慢性疾患をもつ人がセルフケアを行い病状増悪を予防することに向けて、災害後慢性期に被災地外から支援に入る看護職が活用できる健康支援活動の展開モデルを構築することである。2024年度は、1)外部から支援に入る看護職が健康支援を展開するための活動体制を構築する方略を明確化する。2025～2026年度は、2)慢性疾患の増悪や合併症の発症につながるハイリスク者を査定するためのアセスメントツールの開発及び検証を行う。2027年度は、3)ハイリスク者に必要な看護支援を提供する方略を明確化し、1)～3)を統合したシームレスな健康支援活動を展開するためのモデルを開発する。
