Outline of Research at the Start

本研究の目的は、介護老人保健施設入所者が、最期までその人らしく生きることを支援するために、①家族の意向も踏まえた本人のアドバンスケアプランニング（advance care planning：以下ACP）を促進する支援プロセスを明らかにすること、②人生の最終段階にその意向を実現するための施設関係者間および病院施設間の医療介護連携のあり方についてらかにすることの2点である。
本研究は、本人の意向を中心にしたACPの実装をプロセスという時間軸でとらえ、療養の場を跨いで看取りケアの質向上に寄与する、介護老人保健施設におけるACP支援ツールの開発へつなげていくことを目指すものである。
