Outline of Research at the Start

本研究では、超高齢社会の日本と、まもなく超高齢社会となる韓国・台湾・シンガポールにおける高齢者の意思決定支援について、各国に共通する倫理的・法的課題や、各国に特徴的な優れた実践を明らかにし、これらをもとにとりわけ日本における望ましい死の迎え方・備え方を検討し、それを実現する体制の整備への道筋を示すことを目的とする。研究方法としては、包括的な意思決定支援に関する法律を有する英国を一つの参照点としつつ、アジア圏で日本と共通する課題が多いと考えられる韓国・台湾・シンガポールを日本の比較対象とし、法制度の比較研究や倫理的・法的課題の検討などにより、高齢者の意思決定支援の課題の解明を行う。

Outline of Annual Research Achievements

本研究の初年度においては、東アジア諸国における高齢者の意思決定支援に関する倫理的・法的課題の把握に向けた基礎的な調査・研究活動を進めた。
2024年11月21日から23日にかけては、プロジェクトメンバーによる台湾現地調査を実施し、国立台湾大学病院倫理センター、仏教慈済基金会、栄民総病院などの機関を訪問した。これらの訪問を通じて、台湾における終末期医療、特に緩和ケアやアドバンス・ケア・プランニング（ACP）に関する制度や文化的背景についての理解を深めた。また、医療現場における意思決定支援の運用実態や課題について、関係者からの聞き取りを行い、今後の比較分析に資する実践的知見の収集を進めた。
さらに、研究成果の一環として、日本医師会総合政策研究機構のワーキングペーパー「日本・韓国・台湾の終末期の治療中止に関する法政策の比較 -超高齢社会における議論の構築に向けて-」が掲載された。各国における法政策の違いや共通点を明らかにし、高齢社会における終末期医療に関する倫理的・法的課題の比較検討を行っている。
初年度は、今後の国際比較研究を進めるための基礎固めの期間として、実地調査と文献研究の双方からのアプローチにより、研究の方向性と課題を明確化することができた。

Reason

2024年度は、台湾の医療機関を訪問し、終末期医療や意思決定支援に関する現地調査を実施した。
また、日本・韓国・台湾・シンガポール・英国の法制度や実践に関する文献収集と整理を進め、各国の課題や特徴に関する基礎的な情報を蓄積している。今後の比較分析や意識調査に向けた準備も継続中である。

Strategy for Future Research Activity

2025年度は、引き続き、海外における現地調査および専門家との意見交換を進め、各国の制度的・文化的背景を踏まえた比較分析を深める。
また、一般市民への意識調査の実施に向けた準備を整え、得られた知見をもとに、意思決定支援のあるべき姿や制度整備に関する提言の基盤を築く。成果は、国内外の学会や論文等を通じて発信していく予定である。
