Outline of Annual Research Achievements |
前年度に引き続き、名古屋大学医学部附属病院神経内科に通院し、El Escorialの診断基準でprobable以上であり、頭部MRIの撮影が可能である孤発性筋委縮性側索硬化症(ALS)115例に、本研究の趣旨・方法などを説明し、文章によりインフォームドコンセントを得て、3.0テスラ頭部MRIを撮影した。全例MRI撮影時に、臨床評価項目としてALS Functional Rating Scale-Revised(ALSFRS-R)を用いたスケール評価を行い、さらに直近に検査を行った73例については、半年後のALSFRS-Rを再評価した。中途死亡例については、その原因も含め探索することで、全例半年後の状態を確認した。Voxel Based Morphometry (VBM)による脳容積測定(volumetry)とともに、拡散テンソルについては今年度新たにTract-Based Spatial Statistic(TBSS)を用い、Fractional Anisotropy(FA)を用い定量化した上で白質繊維障害を評価した。結果として、ALS群では登録時のvolumetryやFA低下はそれぞれコントロール群に比べて、中心前回や錐体路周辺の他に、前頭葉や基底核にも萎縮とFA低下を認めた。またALSFRS-R低下の速い群では遅い群と比べ、脳萎縮や白質変性が運動野のみならず、前頭葉や基底核にも早期から萎縮とFA低下を認めていること、また加齢に伴いその萎縮・変性がより強く出現していることが判明した。このことから、多軸高磁場拡散テンソルとfMRIを組み合わせた、ALSの大脳灰白質並びに白質の脳内ネットワーク解析を行なった。
