Outline of Annual Research Achievements

アレルギー性鼻炎は喘息と高率に合併し、喘息発症いんしのみならず難治化因子としても知られている。上気道および下気道の炎症は連動して推移し上気道で産生された炎症制御因子は、全身循環を介して喘息に影響を及ぼすことが示唆されている。しかしどの程度アレルギー性鼻炎が喘息の悪化に影響をあたえているかは不明である。そのためアレルギー性鼻炎について鼻炎症状スコアを用いて重症度を判定し、喘息の症状スコア、桐生閉塞、呼気NO濃度で評価した既往炎症との関連を検討した。結果として、アレルギー性鼻炎合併喘息症例では喘息のみの症例に比べて喘息のコントロールの悪化路下気道の炎症の亢進を認めた。さらに今回、喘息のコントロールの悪化と下気道炎症の亢進はアレルギー性鼻炎の重症度と関連することが判明した。このため喘息患者において、簡便な質問票やVASスケールを用いてアレルギー性鼻炎の活動性を判定することは、アレルギー性鼻炎が喘息のコントロールに及ぼす影響を評価するための有用な指標となりうる、と考えられた。またアレルギー性鼻炎ｎ血合いする点鼻ステロイド薬の投与が喘息にどのように影響を及ぼすかはいまだ一定の結論が得られておらず、喘息のコントロールが不完全な中等度・重症持続型鼻炎合併喘息患者に対し、点鼻ステロイド治療の介入による上位同からの炎症制御因子産生の抑制が末梢血好酸球数、喘息症状、肺機能、気道炎症に及ぼす影響を系統的に検討した。点鼻ステロイド薬の治療によりアレルギー性鼻炎の症状のみならず、喘息の症状、肺機能、既往炎症の改善が認められた。また下気道の炎症の改善には末梢血好酸球の抑制の関与が考えられた。今後喘息のコントロールが不完全な患者における独立の規定因子のさらなる検討が必要である。
