Outline of Research at the Start

頭頸部癌再建手術では、高率に手術部位感染(Surgical site infection: SSI)を生じ、患者QOL低下と医療経済的観点から予防策構築が望まれる。頭頸部癌再建手術における感染予防抗菌薬としてβラクタマーゼ配合ペニシリン系抗菌薬の48時間投与の妥当性が明らかにされたが、経験的に行われる術中創部洗浄、口腔ケア、栄養管理等のSSI予防策は未だ科学的根拠が示されていない。本研究課題ではSSIを創部細菌叢のdysbiosisと捉え、創部における原因菌の選択・競争・適応の3過程を遮断するために科学的根拠に基づいた複数の対策を同時に行う「バンドル」構築を目指す。
