Outline of Research at the Start

前研究では、アクションリサーチ手法を用い、「精神科病院と訪問看護ステーションのス
トレングス焦点型移行期支援モデル」を作成した。
今回は、まず、この「精神科病院と訪問看護ステーションのストレングス焦点型移行期支
援モデル」を基に地域移行期に関わる多職種の支援と影響要因を実態調査し、分析する。
次に、地域移行期に生じやすい病状の悪化を防ぐオンライン型協働クライシスプランを作成
し、これを加えて改良した「ストレングス焦点型移行期支援モデル」の検証を行う。
