Outline of Research at the Start

本研究の目的は、成人～高齢期の地域住民に向けた「自分らしく地域で生きる認知症早期ACPプログラム」の構築とその汎用性の評価である。本プログラムは、1）認知症とACPの啓発教材、2）認知症になっても暮らし続けるための地域シート、3）自分の意思や価値観等の必要な情報を対話形式で整理できるACPシート、4）認知症の原疾患の種類と進行時期に応じた継続支援が行えるステップガイドを含むものとする。これらを、地域住民同士が話し合いながら使用できる様式とすることで、地域で認知症の人が暮らし続けるための方策を自分事として考えることに繋がり、認知症になっても共生できる地域を育む一助となることが期待できる。
