Outline of Research at the Start

認知症当事者と家族の生活計画に影響を与えるとされる診断後の早期支援について先行文献と当事者への調査から必要とされる早期支援の項目の選定を行う。選定された項目から看護師を対象とした継続診療の場でも使用できる認知症診断後の早期支援ツールの開発を目指す。ツール開発は、医療職だけではない多職種、当事者、家族が参加し、繰り返しの話し合いを通して行っていく。開発したツールを実装しその実装可能性の評価を行う。
