医療費の削減や患者のQOL向上の観点から外来で化学療法を受けるがん患者が増加してきている。治療後の管理の場も病院から在宅へシフトしてきており、外来看護と訪問看護がケアの役割や責任をシェアする双方向型の連携が求められているが、がん患者が継続的にケアを受けられる連携体制は未だ確立されていない。本研究では、外来看護師と訪問看護師の連携を強化し、外来化学療法を受けるがん患者の継続的支援に向けたシェアードケアモデル（SCM）を開発することを目的とする。ステークホルダーへのニーズ調査、既存のSCMのレビュー、国外の先駆的なSCMの視察調査を通して、日本版SCMを開発する。
