Outline of Research at the Start

本研究では、糖尿病性腎症重症化予防プログラムの推進するため、透析導入ハイリスク患者の抽出に開発されたInformation and Communication Technology(ICT)プログラムである「疾患管理テンプレート」を、県行政区内の複数の医療施設に導入し、三次医療圏レベルで透析導入ハイリスク患者の抽出と医療機関(看護師）主体の行政(保健師）との連携による透析予防指導が、地域の透析導入患者減少効果を示すか検証する。
