Outline of Research at the Start	社会的孤立状態ある高齢者に対する支援では、対象者を支援者や社会資源に早期につなぐことが重要である。その役割を担うのがリンクワーカーであり、我が国では保健・医療・福祉・介護の専門職、生活コーディネーター、民生委員等が専門性や強みを活かした支援を行っている。しかし、支援における判断や支援内容、支援への考え方を明らかにしたものはなく、活動の指針となるものも見当たらない。本研究の目的は、質的記述的研究によりリンクワーカーが活動を展開する過程を明らかにすることである。これにより、リンクワーカーの専門性や立場を超えて共通するアセスメントの視点、優先すべき支援、社会的孤立支援に対する考え方を明らかにする。
