Outline of Research at the Start

介護支援専門員(以下、高齢㎝）と相談支援専門員(以下、障害㎝）が担当するケースには8050問題世帯（80代の高齢の親と50代の中年の子ども両者に専門職による介入が必要な世帯。以下、「8050問題」）があり、世帯単位で考慮した支援が必要となる。両㎝の実践において、相互の連携不足の課題が指摘されている。「8050問題」への連携の土台となるツールとしてケアマネジメントにおけるアセスメントおよびケアプラン作成の実践がある。本研究では、「8050問題」への両㎝のアセスメントおよびケアプラン作成、相互の連携の実践、実践に影響する要因を明らかにする。
