Outline of Research at the Start

本研究は、心不全患者の再入院および重症化予防に向けた症状増悪徴候を早期に察
知し急性期病院、在宅医療機関、訪問看護間の迅速な情報共有を可能とするシステムを活用
したシームレスな心不全管理の効果検証をする。
まず心不全徴候スコアリングの妥当性の検証を行い、心不全増悪徴候の感度、特異度を評価する。その結果を踏まえ、心不全増悪対応システムのパイロットスタディとして、実臨床における訪問看護や病院等で本システムを使用し、実現可能性の評価を行う。
