Outline of Annual Research Achievements

本研究では、自己リンパ球を生体外にて免疫制御性リンパ球へと誘導し、この細胞を体内へ投与する細胞治療を行うことで、免疫抑制剤服用中である肝移植後レシピエントの免疫抑制剤減量・中止を図り、安全な免疫寛容誘導法を確立するため、
1) 生体肝移植術後レシピエントを対象として細胞治療を行い、その免疫抑制剤減量や免疫寛容誘導効果の検討する、
2) 誘導した免疫制御性リンパ球を解析し、その免疫抑制メカニズムの解明する、
3) 細胞治療を行ったレシピエントの末梢血リンパ球やグラフト組織を解析し、細胞治療による免疫寛容誘導作用のメカニズムを探求する、ことを目的として研究を推進している。
本年度は、上記1) 細胞治療を用いた免疫寛容誘導効果を検討する臨床試験を実施するため、新たに施行された「再生医療等の安全性の確保等に関する法律」に則り、必要な諸手続きを進めてきた。また、上記3) 免疫寛容誘導作用のメカニズムを解明するため、細胞治療を行った症例の末梢血リンパ球を経時的に解析した。細胞治療後は早期にCD25+Foxp3+もしくはCD127lo Foxp3+制御性フェノタイプを有するCD4+T細胞が末梢血中で著増していることが確認された。更に、これらの制御性フェノタイプを発現するCD4+T細胞は、治療約6ヶ月後には治療前値に復したものの、以降は再漸増して高めに維持されている事が判明した。免疫寛容誘導の成否との直接的な関連性は不明であり、今後更に検討が必要である。
