Outline of Final Research Achievements

Pathophysiological changes in Alzheimer’s disease (AD), the main cause of dementia in the elderly, start decades before the onset of dementia. Once dementia occurs, there is great variety in the severity and rate of disease progression among individuals. However, contributing factors affecting the process of cognitive decline are still unclear. In order to clarify changes in various biomarkers of AD and the aging process, a cohort observational study is currently underwent. Our first cross-sectional baseline study indicated that severity of hippocampus in MRI morphometry, and increases in CSF phosphlylated tau and high molecular amyloid β oligomer were most sensitive for diagnosis of AD even in the elderly.

Academic Significance and Societal Importance of the Research Achievements

高齢者地域にはアルツハイマー型認知症などすでに認知症を発症している患者が多数存在しており、通常の治療に加えて介護を含めた生活支援が重要である。そのような環境では、認知症の重症度と予後予測のために最も感度の高い指標を判定することは意義がある。認知症の治療と生活支援の計画作成に際して、我々の考察したバイオマーカーを用いた予後予測が有用となる可能性がある。さらに近い将来、認知症治療として脳内の病理過程に作用する治療薬、いわゆる疾患修飾薬、が登場すると言われている。これらのバイオマーカーは疾患修飾薬が臨床応用された時の治療効果判定の指標として有用となる可能性もあり、その点でも本研究は学術的意義がある。
