Research Abstract |
Regional cerebral blood flow (rCBF) is measured by the mew single photon emission computed tomographic device (SPECT) with inhalation of 133-Xe in normal volunteers and patients suffered from cerebral disease. The aim of this study is to assess the normal pattern and several cerebral diseases. There are no remarkable differences among the normal values of the CBF obrained by this device and reported in the literature. Acetazolamide is given intravenously to investigate the state of cerebral vessels. This drug inhibits carbonic anhydrase and increase the CBF resulted from dilatation of cerebral arteries. The value 32% was obtained as a normal increase ratio of CBF in our acetazolamide study. In patients with internal carotid artery occlusion, the mean CBF is decreased on the affected side and normal on the non-affected side. But, the vessels are dilated abnormally not only on the affected side but also on the non-affected side. The extracranial-intracranial bypass surgery is effective for cases of cerebral ischemia caused by hemodynamic factor, and the measurement of CBF with acetazolamide study is useful to find appropriate candidates for bypass surgery. In patients with brain tumor, the mean CBF is increased and the vessels are dilated on both sides. The CBF abnormalities are seen in the remote region from tumors in some patients. This is the first report about the presentation of this remote region via SPECT especially. In patients with hydrocephalus, there is no change in the CBF between before and after ventricle-peritoneal shunting. In conclusion, the combination of 133-Xe inhalation method and SPECT is a great important investigation in several cerebral diseases.
