Research Abstract |
This study was designed to define the procedures and the indications of surgical therapy for the cardiovascular lesions due to KAWASAKI disease. Medical institutes presented as following; 1. Some cases without any symptom had severe progressions of coronary lesions and caused sudden deaths. 2. poor results of medical treatments were observed from those having coronary aneurysms with diameters larger than 10 mm. Surgical investigations showed that the number of operations for KAWASAKI disease were 62, coronary artery bypass grafting (CABG) were 58 and surgery for mitral valve lesion 7. There were no operative death and 5 late post-operative sudden deaths in cases of CABG. The importance of post-operative medical therapy following CABG was demonstrated in this study. In CABG for children, the choice of graft material and the patency of bypass grafts are controversial. This study showed that internal mammary artery graft (IMAG) was adopted in 55 % of cases of CABG and saphenous vein graft (SVG) in 45 %. Late post-operative patency of the IMAG and the SVG were 83 % and 53 % (mean 64 %), and the IMAG showed the tendency of better patency than the SVG. Further study should be required to demonstrate the definitive results of the late post-operative patency of the IMAG. Some institutes suggested a view of indications of CABG for KAWASAKI disease as follows; 1.coronary stenosis of left main trunk, 2.triple vessel disease, 3.double vessel disease, and 4. those having a coronary stenosis concomitant with large aneurysm. This idea could not obtain common consensus of members of this study. Surgery for mitral regurgitation showed no operative death and improvement of symptoms. These results signify the value of the surgical therapy. Further study should be required to decide the indication of the surgery.
