Research Abstract |
Aggregating cells in the affected foci in the lymph node of histiocytic necrotizing lymphadenitis, which affects usually Japanese and rarely reported in Caucasian, are mainly lymphocytes and and histiocytes. Almost of the lymphocytes consisted of T cells of both suppressor (CD <8^+> , Leu <15^+> ) and helper/inducer (CD 4) with the predominance of the former in the early stage and of the latter inthe later stage. After 60 days from onset the ratio of both subsets returned nearly the same as control ones. About 20% of the lymphocytes reacted with Ki67, a marker of cell proliferation and the proportion of the proliferating cells was slightly higher than that in the surrounding lymphoid tissue. No increase of natural killer cells and components and their receptors. The change of population of T cell subsets in process of time resembled those in protracted hypersensitivity reaction. As such reaction may sometimes be induced by viral infections, serum activity of 2'5' oligoadenylate synthetase, which relate to alpha interferon and elevates in acute of viral infection, was detected. The cases within 10 days after clinical onset demonstrated high activity of the synthetase. Appearance of tubuloreticular structures in lymphocytes, histiocytes and capillary endothelial cells, which may relate viral infection or activity of the cells, in affected foci on electron microscopy and the elevation of the activity of synthetase suggested strongly the relation to viral infection. To resolve viral nature of the lesion and the problem of high incidence of the lesion in Japanese gene analysis and investigation of HLA typing seemd to be the most useful methods.
