Research Abstract

A nerve undergoing Wallerian degeneration swells transiently. In a nerve passing through a narrow bony canal, swelling of the degenerating fibers provokes intracanalicular pressure increase; this pressure increase then compresses the initially uninvolved nerve fibers and causes ensuing degeneration. The purpose of this research project was to investigate submandibular salivary ph as anearly prognostic method for peripheral facial paralysis.
1) Salivary pH and flow in control individuals
Submandibular salivary pH at rest was 6.308. The pH elevated steeply as the flow increased and reached the maximum value of 7.388 in 150 sec. In gustatory stimulation, pH reached 1.308 difference from the resting pH (p<.001)in 60 sec.(Effective test economy time).
2) Salivary pH and synkinesis.
Appearance of synkinesis at 6 months from the onset of the palsy was well correlated with pH difference smaller than 0.4 between the involved and uninvolved sides (p<.001).
3) Facial nerve swelling after damage.
Swelling of the nerve was studied chronologically. In partially sectioned groups, the swelling was less degree to produce ensuing secondary degeneration.
4) Pressure in the facial nerve.
A servo-nulling system was used for the interstitial fluid pressure measurement. The pressure was dependent on that of the CSF. It decreased with hypertonic diuretics. The fact suggests that hypertonic diuretics might be effective for reducing the initial swelling of facial paralysis.
