Research Abstract |
VARIOUS KINDS OF VASOACTIVE DRUGS HAVE BEEN INTRODUCED INTO IMPOTENCE THERAPY; AMONG THESE PAPAVERINE HYDROCHLORIDE IS ESPECIALLY EFFECTIVE. HOWEVER,LITTLE FUNDAMENTAL WORK HAS BEEN DONE CONCERNING THE MECHANISM OF PAPAVERINE HYDROCHLORIDE AS A THERAPY FOR IMPOTENCE. IN LIGHT OF THE PAUCITY OF INFORMATION IN THIS RESEARCH AREA WE UNDERTOOK A TRACER STUDY, USING Xe-133,OF THE BLOOD FLOW IN THE CAVERNOUS BODY OF THE PENIS INJECTED WITH PAPAVERINE HYDROCHLORIDE. IN THIS STUDY THE PERIPHERAL BLOOD LEVELS OF PAPAVERINE WERE ALSO MEASURED AT VARIOUS INTERVALS AFTER ADMINISTRATION INORDER TO FOLLOW ITS ELIMINATION FROM THE PENIS AND THUS TO DETERMINE THE RELATIONSHIP BETWEEN BLOOD PAPAVERINE LEVELS AND THE INTENSITY OF PENILE ERECTION. WE FOUND THAT THE PATTERN OF BLOOD FLOW BASICALLY DID NOT DIFFER BETWEEN PAPAVERINE-INDUCED ERECTIONS AND PHYSILOGICAL ERECTIONS. ON THE OTHER HAND, THERE WAS A TENDENCY NOTED FOR LESS PAPAVERINE TO ENTER THE PERIPHERAL BLOODSTREAM IN COMPLETE THAN WAS FOUND IN INCOMPLETE ERECTIONS. IN OUR SERIES OF 177 PATIENTS WITH IMPOTENCE, PAPAVERINE WAS INJECTED INTO THE CAVERNOUS BODY A TOTAL OF 224 TIMES, INDUCING COMPLETE ERECTION IN 61.3% OF THE PATIENTS, INCOMPLETE ERECTION IN 19.4% AND LITTLE OR NO ERECTION IN 13.3%. A DROP IN BLOOD PRESSURE WAS NOTED IN ONE PATIENT AS A SIDE EFFECT FROM PAPAVERINE, BUT IT WAS SOON RESTORED TO NORMAL WITH THE ADMINISTRATION OF HYPERTENSIVE AGENT. FOUR PATIENTS EXPERIENCED PRIAPISM AFTER THE ADMINISTRATION OF PAPAVERINE, BUT THE ERECTION SUBSIDED WITH THE INJECTION OF THE VASOCONSTRICTOR METARAMINOL INTO THE CAVERNOUS BODY. SINCE A WAY HAS BEEN FOUND TO TREAT PRIAPISM, SHOULD IT OCCUR, VASOACTIVE DRUGS MAY NOW BE FOUND SAFER FOR CLINICAL USE IN THE TREATMENT OF IMPOTENCE.
