Research Abstract

We have studies the plasma somatomedin C (SM-C) levels in 101 patients with short stature, who responded normally to GH provocative tests. Thirty-one of them showed decreased plasma SM-C levels. Therefore, we studied the structual abnormalities of GH in these patients by measuring receptor activities and biological activities. Four of 31 cases showed dectrased receptor activities and 2 of 4 showed decreased biological activites. By SDS/PAGE analysis, hGH immunoreactivity in these patients observed in the larger molecular weight portion as compared to the useual 22K portion. Hewever no difference was observed by isoelectricfocusing analysis as compared to plasma obtained from normal child.
We have investigated the SM-C receptor in two short-statured with Type A diabetes, who showed normal GH response and had elevates plasma SM-C levels. The numbers of SM-C receptor of erythrocyte were dectrased. These data suggest that hyperinsulinemia might affect the decreased number of SM-C receptor, which caused the short-stature of these children.
We habe studied 24h plasma GH profiles in 25 normal short children and 4 patients with hypopituitarism. Ten of 25 showed mean 24h plasma GH values less than 3ng/ml. We have also measured 24h urinary GH values and observed good correlation between 24h urinary GH values on one hand and mean GH values (r=0.81, p<0.001) and SM-C (r=0.67, p<0.001) on the other. Thus 24h urinary GH values reflect well endogenous GH secretion. Therefore, the measurement of urinary GH values might be useful for evaluating endogenous GH secrtetion.
