| Research Abstract |
We investigated the physiological responses to dental procedures of the aged patients. Results and conclusions are as follows ;
1. It is difficult to speculate about intra-operative circulatory changes from our routine pre-operative evaluation in the aged patients with hypertension.
2. There are some cases of marked change in blood pressure by impression taking or only opening the mouth.
3. Epinephrine decreased total peripheral resistance and increased cardiac output remarkably, but induced slight change in blood pressure and heart rate.
4. Epinephrine produced significant increase in total peripheral resestance when pretreated with propranolol.
5. It is difficult to speculate about intra-operative circulatory changes from pre-operative blood pressure or rate pressure product.
6. Diazepam suppresses the increase in systolic blood pressure and myocardial oxygen consumption by epinephrine.
From the above results, it is suggested that there are no special method in the general management of the aged dental patients. We must evaluate significance of the patient's complication, degree of the patient's anxiety and detrimental degree of the dental procedures in every patient, and make up the counterplan for mental and physical stresses or vasoconstrictors contained in local anesthetics. General management consists of psychosedation, monitoring and using the vasodilators.
