Outline of Annual Research Achievements

大腸内視鏡で腫瘍性ポリープを摘除することで、大腸がん死を約60%減少できることが立証されているため、腫瘍/非腫瘍を内視鏡で判別することが重要視されている(Zauber, et al. N Engl J Med 2012)。しかし、大規模研究において、医師による診断精度が80%台に留まっている事が指摘されており、非腫瘍性のポリープに不要な治療がされている現況にある(Ladabaum, et al. Gastroenterology 2013)。この過剰治療により、例えば米国では年間40億円の医療費が浪費されており、医療経済的な問題となっている(Hassan, et al. Clin Gastroenterol Hepatol 2010)。
この課題を、人工知能(AI)で解決するという発想のもと、申請者は科研費補助の下、医工連携研究(昭和大-名大)を行い、超拡大内視鏡(CF-290ECI, オリンパス社)を用いて、腫瘍/非腫瘍の鑑別能が90%超の自動診断システムの開発に成功(Mori, et al. Gastointest Endosc 2015)した。
本課題では開発した自動診断システムの臨床での実用性を評価するために、700人以上の患者に実際にAIを用いた内視鏡診断を行う、大規模な前向き試験を実施、90%以上の精度が担保されることを実証した(Mori, et al. Ann Intern Med 2018)。
