Outline of Annual Research Achievements

ヒトや動物が運動を行うとき、複数の関節や筋を協調して動かす協調関係(シナジー)が見られる。このような運動中に観察される運動要素の協調関係は、動作生成において冗長な身体を動かすための低次元表現となっており、脳内身体表現を反映していると考えられる。シナジーは脳卒中及び小脳疾患によってそれぞれ特徴的に変化すると知られており、シナジーを指標とすることで効果的なリハビリテーション(リハビリ)の構築が期待できる。本研究では、神経障害に伴う運動機能低下とシナジー変化の関係を明らかにし、リハビリに応用することを目指して研究を行ってきた。本年度は(1)小脳梗塞ラットの歩行動作解析による運動機能低下のメカニズムの解明、(2)脳卒中患者の筋シナジー解析によるシナジー解析のリハビリへの応用を目指して研究を行い、以下の研究結果を得た。
(1)小脳中間部に小脳梗塞を生じたラットに後肢2足で歩行させ、歩行中のシナジーを解析した。小脳梗塞前と梗塞4日後、7日後、11日後のシナジーを解析し、比較をした結果、小脳梗塞によってシナジーの時間タイミングが変わり、時間が経つに従って(回復に従って)徐々にタイミングが戻っていくことが分かった。また、この変化は足の回転運動に特異的に影響を与えた。これにより、小脳中間部の梗塞が運動機能を低下させ、その後回復に至る過程を定量的に明らかにした。
(2)脳卒中患者の回復の評価に使われるFMA中の筋シナジーを解析することで、FMAに使われる筋シナジーがそれぞれ異なる身体部位の筋活動によって構成され、FMAではこれらの筋シナジーを順に評価していることを明らかにした。また、健常者のシナジーと患者のシナジーを比較することで、シナジーの融合度に患者の重症度が反映されていることを示した。シナジーを重症度の評価に使うことで、(患者へ負担が少ない)単純なタスクで回復を評価できることを示した。
