Outline of Annual Research Achievements

【目的】Ｃ型肝炎ウイルス(HCV)は200万人と感染者が多く、予後も悪い。HCV感染の大きな特徴として感染後80～90％の高率で持続感染化し、慢性肝炎、さらに肝細胞癌を発症することがあげられる。HCV感染ヒト肝臓組織ではHCVが持続的に複製し、ウイルスが完全には排除されず免疫寛容状態になっている。この免疫寛容獲得から破綻に至る機序を解明することを目的とする。
【研究内容】、HCV遺伝子をスイッチング発現するトランスジェニック（HCV-Tｇ）マウスを作製し、急性肝炎から慢性肝炎状態に移行し、肝細胞がんを発症させることに成功した。この慢性肝炎に対する治療ワクチンとしてHCV蛋白質組み換えワクチニアウイルス（HCV-rVV）を作製し、慢性肝炎状態のHCV-Tｇマウスを用いてHCV-rVVの治療効果を検討した。その結果、HCV-rVV接種後１週で慢性肝炎の病態が改善し、肝細胞の膨化、索状配列の乱れ、脂肪変性、グリコーゲン変性といった組織異常の正常化がみられた。rVV接種後4週でrVV-N25接種群のみで肝臓HCV蛋白量が減少しており、肝臓内のNS2特異的CTL 数の増加が認められた。そこで抗CD8抗体および抗CD4抗体を予め投与した状態のマウスにｒVV-N25を接種すると、どちらもHCV蛋白の減少はみられなくなった。このことから、ｒVV-N25のHCV蛋白の排除にはCD４およびCD8＋T細胞が重要であることが示唆された。rVV-N25接種によりHCV蛋白の減少および慢性肝炎の組織像が著明に改善し、これは抗原特異的CTLの誘導のみならず肝臓内へのマクロファージの浸潤が抑制されTNF-aやIL-6の産生が低下したことに起因していると考えられた。これらの知見はrVV-N25が抗ウイルス作用だけでなく抗炎症作用も有することを認め慢性肝炎に対する治療的ワクチンとしての可能性が示された。
