Outline of Annual Research Achievements

膠芽腫は手術後に放射線化学治療を追加しても平均生存期間は約１年であり、もっとも悪性のがんの一つである。初期治療により画像上腫瘍が消失していても、多くの場合1年以内に腫瘍摘出腔周囲に再発してくる。これは腫瘍摘出腔周囲に治療抵抗性で再発の原因となる膠芽腫幹細胞や膠芽腫ニッチが存在することを示唆する。膠芽腫を克服するためには、再発の原因となる膠芽腫幹細胞と膠芽腫ニッチを特異的に攻撃し根絶する必要がある。膠芽腫幹細胞や膠芽腫ニッチで特異的に発現する遺伝子は治療標的になり得る。特に、miRNAは複数の遺伝子を制御しているので治療標的として大変有効と考えられる。そこで、miRNAを利用した膠芽腫の新たな治療法の創出が本研究の目的である。
膠芽腫手術時に腫瘍中心部、境界部、辺縁部（正常に近い部分）の3ヵ所から組織を採取し、それぞれの半分の組織で病理学的検討を行なった後、残り半分の新鮮凍結組織からmiRNAを精製した。組織採取が正確に行なわれた合計9症例においてmiRNAアレイで解析した。また3例においては病理診断を行なったホルマリン固定パラフィン包埋切片から腫瘍中心部、境界部、辺縁部を切り出しmiRNAを精製し解析した。
新鮮凍結組織とパラフィン包埋組織の両者において、組織採取部位によりmiRNA発現が異なり、境界部で特異的な発現を示すmiRNAを複数認めた。腫瘍部新鮮凍結組織とパラフィン包埋組織では比較的同等の発現を認めた。
境界部に特異的に高発現するmiRNAはこれまでオリゴデンドロサイトの分化や癌細胞の増殖抑制にかかわるもののがあり、特異的に発現低下するもには機能解析が進んでいないものが多く含まれ、また代謝にかかわるmiRNAを認めた。
