Outline of Annual Research Achievements

申請者らは、これまでにパクリタキセルの術前化学療法を受ける乳がん患者に対し、5 遺伝子からなる乳がんのパクリタキセル治療応答性診断システムを構築した。しかし、この診断システムの検証のための臨床試験では、パクリタキセルの奏功率を向上させるには至らないという結果になった。一方、パクリタキセル治療症例50 例の遺伝子発現情報を用いたunsupervised クラスター解析を行った結果、5 種のサブタイプ（luminal A, luminal B or C, HER2(＋) , basal-like, normal-like）に分類され、luminal A 乳がんのすべてが、日本乳癌学会による乳がんの病理学的抗がん剤治療効果判定基準のGrade0 -1b と診断され、無効群と判断した。このことから乳がんの各サブタイプでパクリタキセルの奏効率はかなり異なり、サブタイプ毎に治療応答性診断システムを構築する新たな戦略が必要である。しかし、これまで遺伝子発現情報を用いたクラスター解析では、使用する遺伝子、対象とする乳がんによって、同一症例が異なるサブタイプに分類されることもあり、そこで、サブタイプ分類法として新規Lassoクラスタリング法を開発した。また、この新規サブタイプ分類法に基づき分類されたグループの細胞機能特異性を、主成分分析法を中心とした新規解析法を基に検出した。さらに、東大医科学研究所ヒトゲノム解析センターが供給する解析ツール、SiGN-BN（サイン-BN）による解析を進めた。これは遺伝子発現データから遺伝子ネットワークを予測・推定するためのソフトウェアで、ノンパラメトリック回帰によるベイジアンネットワークを用いて観測された発現データから遺伝子間の発現の依存関係（＝遺伝子ネットワーク）を推定する。その結果、サブタイプを分類する候補因子として10種のネットワークを抽出した。
