Research Abstract

本研究の目的は、多数の脳MRIから計算した正常脳構造モデルと形態異常を有する脳疾患画像を比較して、その脳形態の違いを画像統計学の手法で自動的に検出するアルゴリズムを開発することである。目的を達成するために、まず20歳代から70歳代までの1200例の健常日本人脳MRIから計算解剖学的手法(deformation-based morphmetry)により脳形態を変形し、その変形ベクトルの平均値を年代ごとの一例の脳に適用することにより、年代ごとの平均脳(正常脳構造モデル)を計算した。次いで、求めた平均脳形態変形ベクトルの画素ごとの分散を評価するために、ユークリッド距離(EUD)とコサイン類似度(CS)を導入した。ベクトルの分散が大きい程、EUDは大きな値をとり、CSは小さな値を取る。脳領域ごとのEUDとCSの値の分布をみると、後頭葉が最も大きいEUD、小さいCSを示した。このことは、日本人集団では脳形態のバラツキは後頭葉において最も大きいことを示している。この結果は、申請者が別の手法を用いて日本人とドイツ人の脳形態の集団内バラツキを評価した過去の研究の結果と一致している。よって、EUDとCSは平均変形ベクトルの分散を評価できる指標であることがわかった。最後に、画像統計検定を行うための指標としてマハラノビス距離を導入した。60歳代および20歳代の各一例のMRIを、20歳代の正常脳モデルに変形する変形ベクトルをもとめ、そのマハラノビス距離を計算すると、60歳代の脳の側脳室周辺のマハラノビス距離が20歳代のそれを比べて有意に大きいことが示され、統計検定により脳形態差が検出できることがわかった。
以上の研究により、目的とする脳疾患自動診断システムに必要な要素技術の開発をすべて完了した。
