Outline of Annual Research Achievements

昨年までに収集した早産児及び成熟児のアクチグラフ・唾液コルチゾール・尿コルチゾールを解析し，新生児期早期の睡眠覚醒周期が3-4時間程度のアルトラディアンリズム中心であるのに対し，コルチゾール分泌リズムは，生直後には出生時刻に印加された24時間周期のリズムが中心となりつつ，これに重合する形で，胎児型の夕方にピークを迎えるリズムが2週間程度残存することが，多面的なデータから明らかになった．生後1か月・3か月で再建されたアクチグラフでは，ほとんど全症例に明確な成人型昼夜リズムの形成が認められた．今後は新生児期のアクチグラフや内分泌マーカーの変動パターンと，生後1-3か月の変動振幅，そして今後データが蓄積される認知発達との関係を明らかにして行く．唾液検体の収集は，24時間以上の持続は困難であるが，尿中コルチゾールは，紙おむつにサンプル収集用のパッドを装着することで，少ない侵襲で長期間にわたって検体を集めることができた．尿中コルチゾールと血中および唾液中コルチゾールの相関関係も非常に良好であることが証明され，今後の長期フォロー研究における内分泌マーカーの切り札となり得ると考えられた．
脳機能・睡眠・生体リズムの成熟観察と同期して変化する脳の微小構造をベッドサイドで繰り返し観察するために導入した，時間分解近赤外線スペクトロスコピーにより，脳内構造の複雑さを反映する光の散乱係数が，出生までは在胎週数に強く正の相関を見せること，そして，生後は修正在胎週数と相関する上昇を止め，臨床経過と関連した個々に特徴的な増減を見せることが分かった．現在MRI拡散強調画像上の定量値との直接比較を行っており，ベッドサイドでの脳微細構造マーカーとしての有用性が確立されると期待している．
