Outline of Annual Research Achievements |
GPI(Glycosylphosphatidylinositol)アンカーは150種以上の蛋白質を細胞膜に繋ぐ糖脂質でその生合成と修飾に27個の遺伝子が必要である。これらの遺伝子に変異があると重要な機能を担う種々のGPIアンカー型蛋白質(GPI-AP)の細胞膜上の発現が低下し、あるいはアンカーの構造が異常となり、精神発達障害やてんかん、時に高アルカリホスファターゼ(ALP)血症を来す。原因不明の精神発達障害や難治性てんかん患者の中から先天性GPI欠損症(IGD)が次々見つかっている。末梢血のフローサイトメトリー検査でスクリーニング可能であり、GPIアンカー関連遺伝子27個の遺伝子解析で変異遺伝子を同定し、責任遺伝子であるかどうか機能解析で確認できる。今年度はFACS解析とターゲットエクソームあるいは全エクソーム解析によりPGAP3, PIGO,PIGN, PIGT,PIGA,PIGL欠損症が新たに見つかった。そのうちPIGN欠損症は、胎児診断に成功し幸いなことに保因者であったので妊娠を続行した。症例の集積と新たな疾患マーカーの検索により、診断基準の制定と診療ガイドラインの整備を進めている。またビタミンB6(ピリドキシン)がけいれんに有効であることがわかったので現在大阪大学が中心となって臨床研究を進めている。この新しい疾患を広く臨床医に周知するため来年度は小児科学会や小児神経学会にて教育講演を予定しており、さらに最近疾患ホームページを開設してその中で診療ガイドを示している(http://igd.biken.osaka-u.ac.jp/)。将来的には国内外共通の患者データベースを作成する予定である。並行して神経特異的なプロモータNestin-CreとPiga floxマウスを交配したマウス、神経細胞株のノックアウトを作製し, 病態解析を進めている。
