| Outline of Annual Research Achievements |
次世代シークエンサーを用いてHLA 8座（HLA-A、-C、-B、-DRB1、-DQA1、-DQB1、-DPA1、-DPB1）における日本人に代表的なHLA遺伝子全領域の塩基配列を収集した。即ち、20種類のHLA-A、26種類のHLA-C、45種類のHLA-B、41種類のHLA-DRB1、36種類のHLA-DQA1、28種類のHLA-DQB1、15種類のHLA-DPA1、40種類のHLA-DPB1の計251種類の第4区域までのアレル配列を決定した。したがって、本研究にて収集したアレル配列はルーチンタイピングの際の良きレファレンスとして、また今後のHLA多型と疾患や移植成績との関連解析に有用であると考えられた。
再生不良性貧血(AA)を対象にNGSによるHLA対立遺伝子のゲノムDNAおよびRNAを解析した。6p染色体ヘテロ接合欠失(6pLOH)を有する43人の患者を含む312人のAA患者のHLA対立遺伝子喪失頻度を検討した。失われたハプロタイプに含まれるHLA対立遺伝子は、HLA -B * 40：02が最も高頻繁に欠失していた。また、B4002(- )、A(+)の顆粒球におけるHLA-B * 40：02のディープシーケンスの結果、15人の患者すべてにて、フレームシフト、ナンセンスおよびスプライス部位突然変異などの様々な体細胞突然変異を明らかにした。 さらに抗原提示に関与していないHLA-B * 40：02のα3領域におけるミスセンス変異は、B4002( - )顆粒球を有する3人の患者のB4002(+)顆粒球においてのみ検出された。 6pLOHまたは遺伝子突然変異のいずれかまたは両方の結果としてHLA-B * 40：02分子を欠く白血球の顕著に高い罹患率は、HLA-Bアレルを介した造血幹/前駆細胞による細胞傷害性T細胞への抗原提示がAAの発症機序となっている可能性を示唆した。

NGSによる疾患ゲノム解析のためのリソースを整備したこと、疾患ゲノム解析の手法を開発し、その有用性を実証したことからおおむね順調に進展していると考えられる。

NGSによるHLA関連疾患を対象とした解析を推進する。ゲノムDNAおよびRNAの解析に加え、転写制御メカニズムを明らかにするためのエピジェネティック解析の手法も含めたネオ・セルフの理解を目指した手法の確立を目指す。
