Research Abstract

1.動物モデル:(1)LECラット:臨床例と極めて類似の組織像を示し、さらにD-ガラクトサミン(GalN)に対して高い感受性を示すことが判明した。(2)抗ラット肝細胞膜モノクロ-ナル抗体(Mcab):in vitro,in vivoともにMcabにより肝細胞障害が惹起されることが確認された。(3)TNFを用いたモデル:Propionibacterium acnes(P.acnes)あるいはsublethal doseのGalN投与後、TNF投与により広範性肝壊死を作製しえた。さらにP.acnes投与後、non-lethal doseのTNFとIL-lの併用投与でも同様な肝壊死が惹起されることを確認した。2.アラキドン酸代謝産物:(1)ヒトのcycloxygenase,5-lipoxygenase,12-lipoxygenaseと反応する抗体を作製しえた。(2)マウス類壁細胞からPGE,6-keto-PGF,α,TXB_2,LTB_4,LTC_4が産生されることが判明した。3.サイトカイン:(1)ヒトTNFに対するモノクロ-ナル抗体を作製し、新たな高感度の測定系を開発した。(2)P.acnes投与後のマウス肝ならびに脾内マクロファ-ジからは、TNFが、より早期に産生され、ついでIL-lの産生がもたらされることが判明した。さらにP.acnes+LPSによるマウス広範性肝壊死は、抗TNF抗体により阻止しえた。(3)マウス類洞壁細胞からのIL-l産生は、PGE_2、PGI_2により抑制され、それはcAMP上昇を介しての機序が推察された。(4)in vitroの検討より、単独では軽度の障害のみのTNF、IFNγでも同時添加では強い肝細胞障害性が生じることが確認された。4.補体:GalNあるいはMcabによるモデルでは、抗補体剤の投与により、それぞれCH_<50>あるいは肝障害の改善が示された。5.リンパ球亜分画:劇症肝炎患者末梢血ならびに肝組織内にCD8+・11b-細胞を認め、劇症化にキラ-T細胞の関与が示唆された。6.微小循環動態:動物モデルでの検討より、肝細胞障害に先行して類洞内血流速度、ヘモグロビン酸素飽和度の低下が観察された。
