Research Abstract

1.動物モデル:(1)LECラット;ribosomal RNAのメチル化に働くSーadenosylmethionine synthetase活性の肝内での低下が肝障害の成因と考えられ、また肝内への銅沈着がLECラットの基礎的異常であることが判明した。(2)抗ラット肝細胞膜モノクロ-ナル抗体(Moab);Moabによる肝細胞障害の機序としてCa^2の細胞内流入の役割が示唆された。2.アラキドン酸代謝産物:(1)類洞壁細胞からアラキドン酸代謝産物、cytokineが産生され肝局所でのnetworkを形成し、PGがそのhomeostasisの維持に関与しており、その破綻により劇症肝炎が誘導されると考えられた。(2)従来より強力な5ーlipoxygenase阻害作用を有するフラボン化合物がみいだされた。3.Cytokine:(1)scavenger受容体に結合するアセチルLDLによるTNF産生量の増加がみいだされ、劇症肝炎でも同様な機序によりTNF産生がもたらされることが示唆された。(2)臨床例および動物モデルで血中ILー6値の上昇が示された。(3)TNFがIFNγなどの他のcytokineとの相互作用により直接細胞障害性に働き、その細胞障害はapoptosisの様式をとることが明らかにされた。4.補体:GalN、LPSの同時投与あるいはMoabによるモデルにおける抗補体剤の有効性が示された。5.リンパ球亜分画:(1)劇症肝炎患者末梢血ではCDS+・CD11bーのsuprressor T cellの減少、肝組織内ではCD8+・11bーのcytotoxic T cellの増加を認め、劇症化の一部にT細胞の関与が示唆された。(2)劇症肝炎患者血清の健常人リンパ球への添加により、suppressor T cell分画の減少をきたし、可溶型ILー2 receptor(sILー2 R)抗体吸着カラムによる処理で、その減少が解除された。また劇症肝炎患者血中のsILー2 Rの著増がみられ、ILー2のblocking factorとして作用し、T細胞分画の異常をもたらしていると考えられた。6.微小循環動態:動物モデルでの検討より、肝細胞障害に先行して肝小葉内酸素需給動態の破綻が出現していた。
