ヒト胆管上皮抗原の精製と原発性胆汁性肝硬変における自己免疫反応の解析

Research Abstract	原発性胆汁性肝硬変(PBC)の成因にはヒト胆管上皮に対するTーcell auto immunityが重要である事を症例数を増して確認した。1)PBCの脾細胞は自己胆管上皮細胞に対して細胞障害活性を示す。細胞障害活性はTリンパ球に見い出され,non Tリンパ球には認められない。CTLのphenotypeはCO8陽性である。CTL活性はauti MHC class Iモノクロ-ナル抗体で阻害される。2)PBCの未梢血単核球は胆汁中より精製したヒト胆管上皮抗原(B1)に対してproliferation assayが陽性である。芽球化反応はT cell分画に存在する。B1抗原に対するT cell proliferationはPBCに疾患特異性であり,他の肝内性,あるいは肝外性胆汁うっ滞症,自己免疫性肝炎,ウイルス性肝炎では検出されない。B1抗原は家免抗ヒト胆管上皮抗原抗体をプロ-ベに用いた,Western blottingにて分子量〉200K.D.であり,シアル酸染色陽性,脂肪染色陰性である。B1の既知分子との交叉抗原性をWestern blottingで検討すると,MHC class I抗原,MHC class II抗原,lgs,secretory lga,ラミニン,IV型コラ-ゲン,CEA familyであるbiliary glycoprotein,更に抗ミトコンドリア抗体の対応抗原であるPDH E2 ,PDH E3,BCKG・E2とは抗原性を共存しない。以上の成績よりPBCではヒト胆管上皮抗原B1に対する細胞性免疫が成因に関与していると考えられる。現在B1分子のT cell epitopeをT cell clone,in situ hybridizationにより作成したモノクロ-ナル抗体を用いて解析中である。
