滲出性中耳炎における粘液線毛疫の病態生理に関する研究

| Research Abstract |
モルモットに、1)黄色ブドウ球菌、2)エンドトキシン,3)インフルエンザAウィルス,4)ヒスタミンなどの鼓室内注入や、5)放射線照射,6)二酸化窒素暴露などを負荷し、1)以外の因子負荷によって実験的滲出性中耳炎を作製しうることを認めた。また、5)及び6)以外の因子によって惹起される滲出性中耳炎は一過性のモデルであることを確認した。このようなモデル動物の粘液線毛機能と上皮細胞の微細形態の観察成績により、滲出性中耳炎の発症機序には、(1)粘液分泌細胞の増加や機能亢進(2)血管誘過性の亢進による滲出液の増加、(3)線毛活性の低下の3因子が関与していた。このうちで、(1)あるいは(2)の因子に(3)の因子が加重されに場合にのみ、滲出性中耳炎の発症することが認められた。また、(3)の線毛活性に関しては、耳管内及び鼓室口部の線毛活性が特に重要であり他の部位の線毛活性は決定的な因子とならないことも確認された。また5)及び6)の因子負荷によって発生した遷延性の滲出性中耳炎においては耳管内及び鼓室口部の線毛活性の改善は長期間にわたって不良であった。したがって、滲出性中耳炎の治療の標的は耳管内及び鼓室口部の線毛活性の改善にあることが示された。
そこで滲出性中耳炎モデル動物に線毛活性賦活作用を有する薬物を、実際に投与し、滲出性中耳炎の治癒機転を促進しうるか否かを検討したその結果、これまでに2種類の薬物は実験的滲出性中耳炎における耳管内及び鼓室口部の線毛活性を賦活化し、その治癒機転を促進することが確認された。
