| Research Abstract |
肝の大量切除における耐術性或は肝再生率は,残存肝動脈血行に大きく左右されるとおもわれるが,このことは病態肝,移植肝においてとくに重要である。イヌ,ラットを用いた肝大量切除モデルでは,残存肝動脈血行の有無は,耐術性に影響せず,わずかに術後早期の肝エネルギ-代謝の回復に影響するにすぎなかったが,黄疸肝では耐術性,肝エネルギ-代謝の回復が,それぞれ低下或は遅延した。また,イヌ移植肝においても,動脈血行の開存の良否は耐術,エネルギ-代謝回復を大きく左右した。
再建動脈血行の維持は,血栓形成,吻合部肥厚,消化液汚染による再建部の破綻によって妨げられる。血栓形成はheparinよりもむしろmicro surgeryが有効であったが,吻合部肥厚にはheparinが有効で抗血小板薬の初用は否定的であった。消化液汚染に対して有効なfibrin glueも,collagen未混入がその効果を増強するという当初の予想は,ラットを用いた動脈再建モデルでは,混入比0.1%,1%,10%,50%,100%ののいづれにおいても証明出来なかった。
