抗クラスII抗体ex vivo penfusionによる犬移植肝拒絶反応制御の試み

| Research Abstract |
サイクロスポリンAの導入によって肝移植成績は著しく向上し,1年生存率75%,5年生存率50〜60%となったが,これは再移植を行った上での成績である。急性拒絶反応は全症例の80%前後にみられ,特に移植後5〜10日目に出現する急性拒絶反応は予後を大きく左右する因子であり,治療に成功しない場合には再移植が不可欠となる。
ヒト肝移植後の急性拒絶反応については,いまだ十分な免疫学的解析がなされていないが,われわれのイヌでの実験結果からすると,肝類洞壁細胞のうちで,Kupffer細胞および類洞壁内皮細胞上に発現しているクラスII抗原が、急性拒絶反応のinitiation moleculeになっている可能性が高い。
従って,本研究の目的は,イヌ肝移植モデルを用いて,移植肝由来のクラスII抗原を抗クラスII抗体で覆うことによって,肝移植後早期の急性拒絶反応を人為的に操作し,移植肝の生着延長効果の有無を明らかにすることにある。
具体的には,移植前にイヌ・ドナ-肝を抗クラスII抗体でex vivo perfusionし,肝移植を行い,つぎのような研究成果をえた。
1)抗クラスII抗体で潅流後の肝非実質細胞を刺戟細胞としたリンパ球混合培養は有意に抑制されていた。
2)肝移植後の肝機能の推移を潅流群と非潅流群とで比較すると,急性拒絶反応の指標となる血清ビリルビン値,アルカリフォスファタ-ゼ値は,非潅流群では移植後4日目より上昇するのに対し,潅流群では正常域にあり,有意に急性拒絶反応は抑制されていた。
