Research Abstract |
Percutaneous intradiscal laser nucleotomy (PILN) has been studied in our laser laboratory. In the experimental studies, Sacrificed or anesthetized rabbit and goat lumbar discs were used. This experimental study took following process ; 1) to confirm the possibility of nucleus vaporization with laser, 2) to confirm the usefuldlss of this method concerned with histological changes and intradiscal pressure decrement, 3) to confirm the safety with thermography and thermocouples, 4) to develop and improve laser devices, original double lumen needle and tip type pressure transducer for feasibility, 5) to establish evaluation methods with pressure tra'nsducer and MRI index histograms. From 1 to 5 basic studies, the procedure of this method with ND : YAG laser was almost established. And nucleus pulposus absorvance was measured. From absorvance, we guessed that PILN had two type treatment for lumbar disc hernia. The one was nucleolysis and the other was nucleotomy. Now we are starting for clinical use. And intervertebral bone fusion through needle has been studied after PILN.
