| Research Abstract |
本研究者らは、冠攣縮により生体内トロンビン産生の指標であるフィブリノペプタイドAが増加することを証明してきた。今回の研究においては、組織プラスミノ-ゲンアクティベ-タ-(tーPA)およびプラスミノ-ゲンアクティベ-タ-インヒビタ-(PAI)を指標として、線溶系の面から検討を行なった。冠攣縮性狭心症患者および不安定狭心症患者においてtーPA抗原およびPAI活性を調べ、tーPA抗原の増加も認められるが、線溶系の指標であるPAI活性は亢進しており、線溶系は障害され血栓が溶けにくい状態にあることが明らかとなった。また、治療とともにPAI活性は低下し、線溶系の障害が改善されていくことも証明した。また、心筋梗塞の急性期においても、tーPA抗原およびPAI活性を調べ緊急冠動脈造影と対比検討することにより、興味深い知見を得た。すなわち、従来は心筋梗塞急性期にはPAI活性は亢進しているとされてきたが、正常範囲のPAI活性を示す症例もあり、それらの症例では血栓溶解療法前に既に心筋梗塞責任冠動脈が開存している症例であることが判明し、PAI活性と心筋梗塞責任冠動脈の開存性との関連が示唆された。さらに血栓溶解療法の効果とPAI活性の関連について検討したところ、再潅流の得られた群のほうが、再潅流の得られなかった群に比しPAI活性は高値を示した。後者の内の62%は正常範囲のPAI活性を示しており、これらの症例では急性心筋梗塞の冠動脈閉塞の原因として、粥腫内の出血とか局所的な冠動脈解離による機械的な圧迫による非血栓性の要因が考えられた。また、本研究者らは、血管平滑筋を増殖させ動脈硬化との関連で注目されている血小板由来増殖因子に関する臨床的検討も行ない、冠循環における血小板由来増殖因子の血漿濃度は、対照患者や安定労作狭心症患者に比し、不安定狭心症患者において高値を示し、血小板由来増殖因子の不安定狭心症の病態への関与が示唆された。
