SUBCLASSIFICATION OF PARATHYROID ADENOMA WAS ATTEMPTED IN THE FOLLOWING THREE POINTS : 1) EXPRESSIONS OF SUGAR CHAIN OF THE CELLMEMBRANE,2) ANALYSIS OF ONCOGENE,AND 3) CELL PROLIFERATION. 1) THE BINDING SITES OF LECTINS IN PARATHYROID GLANDS WERE DETERMINED BY AN IMMUNOHISTOCHEMICAL METHOD IN NORMAL PARATHYROID GLAND,HYPERPLASIA,ADENOMA AND CARCINOMA.THE USED LECTINS WERE COMMERCIALLY AVAILABLE SBA,CON-A, WGA,RCA,BSA II AND PNA.FOR NORMAL PARATHYROID GLANDS (2 CASES) AND HYPERPLASIA (2 CASES), WGA AND BSA II WERE STAINED IN CYTOPLASM AND CELL MEMBRANE.FOR CARCINOMA (1 CASE), ALL LECTINS BUT BSA II WERE STAINED.IN PARTICULAR,SBA REVEALED STRONGER STAIN THAN HYPERPLASIA AND ADENOMA.FROM THE STAINING PATTERNS OF LECTINS,IT WAS SUGGESTED THAT ADENOMAS (22 CASES) ARE DIVIDED INTO ONE GROUP SIMILAR TO CARCINOMA AND ANOTHER TO NORMAL PARATHYROID GLAND AND HYPERPLASIA GROUPS (DOI ET AL., 1991). 2) TO EXAMINE WHETHER OR NOT A PART OF HUMAN PARATHYROIDAL ADENOMAS HAVE A MALIGNANT POTENTIAL,15 PARA

THYROID TUMORS INCLUDING 13 ADENOMAS,1 HYPERPLASIA AND 1 ADENOCARCINOMA WERE STUDIED FOR THE AMPLIFICATION OF THE C-ERBB2 PROTO-ONCOGENE.ELEVEN ADENOMAS OF THESE TUMORS WERE ALSO EVALUATED FOR THE IMMUNOHISTOCHEMICAL EXPRESSION OF THE C-ERBB2 GENE PRODUCT.THE AMPLIFICATION OF THE C-ERBB2 PROTO-ONCOGENE (2-FOLD OR MORE GREATER THAN CONTROL) WAS RECOGNIZED IN 3 ADENOMAS (23.1% OF ALL ADENOMAS) AND 1 ADENOCARCINOMA AS COMPARED TO A POOLED HUMAN NORMAL PLACENTA,WHICH SERVED AS CONTROL.SIX OUT OF 11 PARATHYROID ADENOMAS (54.5%) REVEALED THE IMMUNOHISTOCHEMICAL EXPRESSION OF THE C-ERBB2 GENE PRODUCT ON THE CELL MEMBRANE AND IN THE CYTOPLASM.THREE ADENOMAS (27.2%) SHOWED THE STRONGLY POSITIVE (++) STAINING AND THE OTHER 3 ADENOMAS (27.2%) SHOWED THE WEAKLY POSITIVE (+) STAINING.THE 2 CASES HAVING THE AMPLIFICATION OF THE C-ERBB2 PROTO-ONCOGENE ALSO REVEALED THE STRONGLY POSITIVE STAINING.THE C-ERBB2 PROTO-ONCOGENE,WHICH HAS BEEN REPORTED TO BE AMPLIFIED OR EXPRESSED IN SOME ADENOCARCINOMAS,MIGHT BE ASSOCIATED WITH THE INITIATION AND PROGRESSION OF SOME PARATHYROID ADENOMAS.THE PRESENT FINDINGS MIGHT SUGGEST THAT SOME PARATHYROID ADENOMAS COULD HAVE A MALIGNANT POTENTIAL (MORIYAMA ET AL., 1992). 3) PARAFFIN-EMBEDDED SECTIONS OF PARATHYROID ADENOMA FROM 27 PATIENTS WITH PRIMARY HYPERPATHYROIDISM WERE EXAMINED FOR THE EXPRESSION OF PROLIFERATING CELL NUCLEAR AN TIGEN (PCNA). PCNA WAS EXPRESSED ON THE NUCLEI OF THE ADENOMA CELLS.THE SIZE OF ADENOMAS WAS NOT RELATED TO THE POSITIVE RATE FOR PCNA, WHILE THE PREOPERATIVE SERUM CALCIUM LEVEL AND POSITIVE RATE FOR PCNA REVEALED STATISTICALLY THE LINEAR REGRESSION (Y=0.42X-3.96, P<0.05). MEAN OF SERUM CALCIUM LEVEL IN THE GROUP IN WHICH POSITIVE RATE OF PCNA WAS MORE THAN 1.0%WAS STATISTICALLY HIGHER THAN THAT IN THE GROUP IN WHICH POSITIVE RATE OF PCNA WAS LESS THAN 1.0% (P<0.05) (KURIMOTO ET AL., 1993). IN SUMMARY,PARATHYROID ADENOMA COULD BE CLASSIFIED AT LEAST INTO TWO TYPES, THE FORMER ANALOGOUS TO THE CARCINOMA, WHILE THE LATTER SHOWING SIMILAR TENDENCY TO THE HYPERPLASIA. Less
