Research Abstract

1.肺及び心肺保存に関する研究:肺保存では、保存液としてリン酸緩衝液とした細胞外液組成液の有用性とCa^<2+>拮抗剤添加の有効性が明らかとなった(藤村・富田)。また、摘出肺灌流モデルを用いた保存肺の機能評価方法が考案され、移植前の機能評価判定に進展が見られた(岡田、富田)。更には、肺保存研究の一環として心停止をきたしたドナ-からの肺移植の研究が進み、一定条件下では可能であることが示唆された(藤村)。一方、心肺保存では、心筋保護液におけるmulti dose投与の有効性が明らかとなった(田辺)。
2.肺及び心肺の機能維持に関する研究:1)拒絶反応に関する研究;拒絶反応早期診断に関する研究では、気管支肺胞洗浄液中のリンパ球の細胞障害活性やIa抗原発現率の検出の有用性が示唆された(山口・富田)。一方、免疫抑制剤としてFK506の効果が明らかとなり、サルによる追試が進行中である(人見、富田、古瀬)。一方拒絶反応の自然寛容モデルが発見され、拒絶反応防止の上で大きな手掛かりが提供された(藤村)。2)移植後病態に関する研究;移植後急性期の合併症である虚血再灌流障害に白血球とニュ-ロぺプタイドであるsubstance Pの関与が指摘されており、その予防効果として好中球モノクロ-ナル抗体やPAF拮抗剤の有効性が判明した(藤村、草川、小松)。また、気管支鏡下にドップラ-制効果による気管支粘膜血流量測定の技術を用いて、気管支吻合部創傷治癒に対するPGI_2の効果や免疫抑剤の影響の観察が可能となった(加藤、人見、田辺)。一方、移植後遠隔期における肺内神経再生の可能性が示唆された(藤村・川島)。
3.移植適応に関する研究:肺高血圧モデルによる肺移植の有用性が明らかとなるとともに、末期心不全患者の詳細な心肺機能評価により、肺および心肺移植の適応が明らかとなってきた(川島、小柳)。
