心疾患ににおける脳性Na利尿ペプチド(BNP)の臨床的意義

| Research Abstract |
ヒトBNPに特異的かつ鋭敏なradioimmunoassay(RIA)を開発し、SepーPak C_<18>抽出法によるヒト血中BNPの測定法の開発を試み、健常人ならびに心不全患者の血中BNPの動態ならびに心カテ-テル時の逆根的静脈サンプリングによりBNP分泌の局在ならびに剖検心でのBNP含量を解析した。ヒトBNPを家兎に免疫してヒトBNPを0.3pg検出可能で、αーnANPとは全く交又性を示さない抗体を用いてRIAを確定した。血中BNPはANPと比べて10分の1と低値であった。心不全患者の血中BNPは心不全の重症度(NYHA)と平行して上昇し、特に重症例(III、IV度)ではANPより上回っ た。心臓カテ-テル法で採取した静脈血中のBNPは冠静脈洞で最も高く、次いで肺動脈、大動脈であった。剖検心の心房および心室にBNPは検出され、その濃度は心室より心房が約20倍高値であった。しかしANP含量に比較するとBNPは約3%であった。心臓抽出物のゲルろ過を行った結果、大部分はBNP(1ー32)に一致するピ-クであったが一部は大分子ピ-クとして溶出した。以上の研究成績からBNPはANPと同様、心臓ホルモンとして循環するが、その濃度は明らかに低い。しかし重症心不全ではBNPの分泌はANP以上に亢進し、心房より心室での生合成が促進されるものと推定される。心臓ではANPがγーANPとして前駆体の形で存在するのに対してBNPのプロセシングは既にγーANPから進んでおり、このプロセシングは不全心で亢進しているか否か、又血中でも分子型の変化が存在するかは今後の検討が必要である。
