| Research Abstract |
母体ならびに胎児循環におけるエンドセリン(ET)およびatrial and brain natriuretic peptides(ANP,BNP)の関与を検討した。正常妊婦のETー1ーLI値は妊娠経過を通じて正常非妊婦の値と有意差を認めなかった。しかし、妊娠中毒症を合併した症例では、正常群に比して有意(p<0.005)の高値を示した。これに対して、妊娠前から慢性高血圧を合併していても妊娠中に悪化しなかった症例では、血中ETー1ーLI値は有意の増加を示さなかった。一方、血管内皮の損傷を伴うと考えられるDICやHELLPでは、高血圧がそれほど重症でなくともETー1ーLIが高値を示した。この結果は、血中ETの上昇が血圧の重症度のみではなく他の因子とも関連していることを示唆していると思われる。また、臍帯静脈血中には、母体血中値の数倍高濃度のETー1ーLIが存在するが、胎児仮死の症例ではさらに高値を示し、胎児hypoxiaで上昇するとされるvasopressin、ANPおよびBNP濃度と良好な相関を示した。これらの所見は、胎児血中におけるエンドセリン濃度の上昇機序が母体血中のそれとは異なっており、主にhypーoxiaに関連していることを示唆していると考えられる。母体血中BNP濃度は非妊婦と同様の値を示したが、ANP濃度は妊娠中期に正常非妊婦の値より高値を示した。重症妊娠中毒症合併妊婦の血漿中のBNP,ANP濃度は、正常妊婦血中値に比してBNPは約7倍、ANPは約3倍高値を示した。血中BNP,ANP濃度は母体血圧と高い相関を示した。妊娠末期正常分娩時に採取した臍帯静脈血中のANP濃度は母体血より高値を示したが、BNP濃度は母体血中値とほぼ同じであった。一方、胎児仮死徴候を認めた症例の臍帯静脈血中ANP,BNP濃度は各々正常新生児血中値に比してBNPは約13倍、ANPは約4倍の高値を示した。すなわち、BNPは妊娠中毒症や胎児仮死に際して、ANPと協調する形で母体および胎児循環の恒常性維持に寄与していると考えられた。
