新しい肺保存液の開発とADF/hThioredoxineのラジカルスカベンジャー効果

Research Abstract	1.ユーロコリンズ液のグルコースにかわりトレハロースが加えられた液で12時間保存犬肺が良好な肺機能を示し、トレハロースの有効性が証明された(Hirata T et al.Surgery,1994)。そこで、ユーロコリンズ液イオン組成で7%或は10%トレハロースが加えられた液を用いて20時間犬肺保存後、左肺移植を行なったところ、7%トレハロースが加えられた液は10%より良好であっり、最適トレハロースの濃度は7%以下と考えられたが、それでさえ充分な肺保存効果を示さず、ユーロコリンズ液イオン組成液の限界が明らかとなった。そこで、クロライドシフト(塩素による水分子の細胞内移動)を来さない陰イオンとしてグルコネートを、膠質浸透圧源にHESを、糖質としてトレハロースを含む細胞外液型ET-K液と細胞内液型IT-K液の2種類の臓器保存液を作成し、20時間の犬肺保存実験を行った。この結果、細胞外液型ET-K液は細胞内液型IT-K液やユーロコリンズ液と比較し有意に良好なPaO2値を示し、良好な肺保存効果を認めた。 2.ADFによる肺再灌流障害の防止:大腸菌レコンビナントで作成した人サイオレドキシン(ADF:30mg/kg)を用いて、ラット肺動脈75分間クランプ実験による再灌流障害の予防もしくは減弱化を検討したところ、ADF投与群で有意にPaO2値が良好であった(H.Yokomise et al.Respiration,1994,accepted)。同様の実験を、兎、犬で行っている。 3.FK506大量投与による犬移植肺の免疫寛容導入:トロント大学と共同で行った。通常使用量の10倍を投与すると、30日間の免疫寛容があったが、それ以後は肺は拒絶された(T.Hirai et al:Ann Thoraic Surgery;1993/09 accepted)。
