Research Abstract	1.妊娠中毒症合併妊婦では母体血中のET濃度が正常妊婦に比して有意に高値を示した。血中ET濃度は高血圧重症例では高値を示した。一方、HELLPなど血液凝固機能異常が著しい症例では、高血圧がさほど著明でなくても高値を示す傾向にあった。すなわち、ETの産生亢進が血管内皮細胞の障害と密接に関与していることを示している。 2.妊娠中毒症合併妊婦血中のANPおよびBNP濃度は、正常妊婦血中値の数倍高値を示した。これらの上昇度は収縮期圧、拡張期圧いずれとも正の相関を示し、ANP、BNPが心臓への負荷にたいして反応性に分泌増加しているものと推測された。 3.胎児仮死例では、ET、BNP、ANPいずれもその血中濃度が著しく上昇していた。特に、pulsed Doppler法で胎児下行大動脈血流の逆流が生じているような症例、すなわち、胎児が心不全の状態にある場合にはANP、BNPの分泌が正常時の5-20倍に亢進していた。すなわち、妊娠末期のヒト胎児では、低酸素症などスストレスに反応してANPやBNPを分泌し自らのの循環動態の恒常性を維持する機能が存在すると推測された。 4.放射能labelしたET-1、ET-3、ANP、BNP、CNP、C-ANFを用いた種々の結合実験の結果、胎盤・羊膜および脱落膜にはETAR、ETBRおよびナトリウム利尿ペプチドの受容体が存在することが明らかになった。ETAR、ETBRに関してはNorthern bolt法にてmRNAの発現も確認された。すなわち、ET、ANP、BNPがこれらの組織で何等かの生理作用を有している可能性が考えられる。 5.培養ヒト羊膜細胞におけるET、BNPの産生分泌を確認し、さらにこれらのペプチドの産生調節機序を検討した。ET産生はTNF、TGF、IL-1の添加で促進された。また、BNP産生はTGFで促進されたが、妊娠末期に羊水中に増加するcortisol、EGFの添加により抑制された。
