顎関節滑液含有のリンフォカイン濃度の測定並びに顎内障画像診断との相関性の検討

| Research Abstract |
顎関節滑液を採取した患者において、滑液中の関節軟骨プロテオグリカン(PG)分解活性とX線的な顎関節の骨変化あるいは関節円板の穿孔の有無との関係を検討した。さらに、関節軟骨の分解の抑制に深く関与していることが明かとなっているtissue inhibitor of metalloprotease(TIMP)の顎関節滑液内の含量とX線所見との相関性を検討した。PG分解活性はウサギ関節軟骨細胞培養系を用いて測定し、IL-1βの基質分解能を基準として定量化した。【1】関節軟骨細胞のPG分解に対するIL-1βの作用:ウサギ関節軟骨細胞培養系のPGを^<35>S硫酸にてプレラベルした後、各濃度のIL-1βを培地に添加して48時間で培地中に放出された^<35>S放射活性から、細胞層に残存したPG量を算定した。この結果、3pg/mlから300pg/mlでIL-1βの濃度依存性にPGの分解が促進し100pg/mlのIL-1βは約50%のPGを48時間で分解することが判明した。【2】顎関節滑液のPG分解能とX線所見との関係:顎関節滑液を関節軟骨細胞培養系に添加してPGの分解能を算定し、X線所見との相関性を検討した。この結果、顎関節滑液中のPG分解活性と顎関節頭の骨変化あるいは関節円板の穿孔との間には有意な相関関係はないことが判明した。【3】顎関節滑液中に存在するTIMP含量とX線所見との関係:顎関節滑液中のTIMP含量とX線的な顎関節頭の骨変化あるいは関節円板の穿孔の有無との関係を検討した結果、顎関節滑液中に含まれるTIMP含量は顎関節の骨変化あるいは関節円板の穿孔とは相関しないことが判明した。
今回の研究では、ウサギ関節軟骨細胞培養系を用いて、PG分解活性測定が可能な実験系を開発した。本系ではIL-1βによるPG分解活性を指標として滑液の基質分解能を定量化できた。顎関節滑液のPG分解能とX線所見との相関性はなかったが、これが病態の時期によるものか、病期の全期間にあてはまることかについては現時点では不明であるが、本実験系を用いて長期的な経過観察を行なうことにより全望を明らかにしうると考えられる。
